PATIENT TERMS AND CONDITIONS

All patient's will require a telephone consultation prior to attending for a face-to-face consultation or treatment.

All telephone consultations are free of charge and without obligation. 

​

All face-to-face consultations for any reason are at additional fee to be confirmed during the telephone consultation. 

​

Patients are not obligated to consent for face-to-face consultation or treatment following the initial telephone consultation. 

​

All initial telephone consultations are free. Face-to-face clinic appointments will be required for the following:

   Management of oro-facial pain
   Pre-operative treatment planning prior to bone grafting procedures and sinus lifting procedures
   Pre-operative treatment planning as part of dental implant consultation

   Assessment of pro-facial lesions prior to performing biopsy

Patients requiring dental extractions for any reason will be required to attend for a face-to-face consultation if radiographs (x-rays) have not been taken, cannot be provided or are deemed to be inadequate for treatment planning purposes for the extraction required.

In cases whereby a patient contacts OM OS for a dental extraction but it is unclear as to the tooth that may require extraction a face-to-face consultation will be required. 

 

Following confirmation of a face-to-face consultation or treatment appointment, the patient shall be contacted by the reception staff at the practice of his/her choice. A non-refundable deposit will be taken in order to confirm the patient's appointment.

​All deposits for face-to-face consultations or treatment appointments are non-refundable. 

​

Face-to-face consultations or treatment appointments may be rearranged on one occasion only without additional charge. Any further rearrangement of a face-to-face consultation or treatment appointment will result in a forfeit of the deposit made for the face-to-face consultation or treatment appointment. The subsequent face-to-face appointment or treatment appointment will be charged in full at the pre-agreed fee.

Payment plans for face-to-face consultations or treatment appointments are not available. All payment is taken in full on the day for the service provided following completion of the service for which a patient may attend for.

Telephone reviews or face-to-face consultations following treatment and/or the management of post-operative complications including but not limited to post-operative infection will be offered without additional fee. However, should a patient be unable to attend OM OS for treatment, he/she may arrange for review at the referring dentists practice. This will be charged additionally at the referring dentists discretion. 

 

All patient's treated under the remit of OM OS shall have his/her treatment limited to the service(s) outlined. All patient's shall be required to liaise with their general dental practitioner/regular dentist for treatment outside the service(s) for which he/she has contacted OM OS for.

Where appropriate patient details will be provided to the dental practice/site where treatment is to be performed using a secure and encrypted platform in order to confirm the appointment date and time. 

​

Where appropriate dental radiographs (x-rays) and additional patient records including but not limited to photographs and biopsy results will be shared with the patient's general dental practitioner/regular dentist using a secure and encrypted platform. This is done so in order to facilitate long-term patient care.

Discharge summaries will be provided to all patient's general dental practitioner/regular dentist following completion of treatment. In cases where a patient does not have a general dental practitioner/regular dentist the discharge summary will be provided to the patient. It is then the patient's responsibility to provide this to the general dental practitioner/regular dentist of their choice. All discharge summaries sent to a patient's general dental practitioner/regular dentist will be provided via a secure and encrypted platform. This is done so in order to facilitate long-term patient care.

OM OS reserves the right to reject patient enquiries for treatment for any reason in addition to those stated above. 

​

All patient enquiries will be responded to within the hours of 0900 - 1700 Monday - Friday. Enquiries made outside of these hours will be responded to during these working hours.

REFERRER'S TERMS AND CONDITIONS

All referral's to OM OS for the services outlined must be made through the referral form​ provided on the homepage. 

​

Referrals for dental extractions including, but not limited to third molar surgery and orthodontic extractions must be made with a plain film radiograph at a minimum. Where indicated, CBCT scans and/or additional 3D imaging referrals shall be made by OM OS. Referrals for dental extractions made without radiographs may be rejected. 

​

Incomplete referrals made using the form provided on the homepage will be rejected. 

​

Telephone reviews or face-to-face consultations following treatment and/or the management of post-operative complications including but not limited to post-operative infection will be offered by OM OS without additional fee. However, should a patient be unable to attend OM OS for treatment, he/she may arrange for review at the referring dentists practice. This will be charged additionally at the referring dentists discretion. Advice may be sought by the referring dentist for management of such post-operative complications.

All face-to-face consultations and treatment appointments will be limited to management of the reason for referral. The referring practitioner shall be informed of any patient enquiries made about any dental condition or treatment made outside of the reason for referral. 

​

All treatment performed will be done so only after a formal referral has been made. 

​

OM OS reserves the right to reject referrals outside of any of the reasons stated above.

All referral enquiries will be responded to within the hours of 0900 - 1700 Monday - Friday. Enquiries made outside of these hours will be responded to during these working hours.
